Skin aging is essentially determined by two factors:

 

  1. Intrinsic (genetically determined)
  2. Extrinsic (related to the external environment: environmental factors, UVA and UVB solar radiation, hormonal factors…).

 

We talk of chronological aging or chronoaging when we refer to those skin alterations linked to biological factors and photoaging when the skin damage is attributable to sunlight.

 

Chronoaging

 

The outermost layer of the skin, the epidermis, thins and therefore no longer plays its role as a barrier correctly. The supporting structure of the skin, the dermis, undergoes important alterations at the level of collagen fibers which thicken and lose their elasticity. The superficial microvascular system weakens and thus increases capillary fragility.

 

Photoaging

The epidermis thickens, dyskeratosis and tumor degenerations are frequent, the density of melanocytes in the skin increases, the collagen undergoes fibrotic alterations, resulting in skin elastosis, microvascular alterations and dilation of blood vessels.

 

In any case, the two types of aging (intrinsic and extrinsic) combine to determine the cascade of events that are responsible for the characteristic appearance of aged skin: wrinkles, spots, capillary fragility, closed cysts and comedones, dyskeratosis and hyperkeratosis.

 

What are the solutions to counteract skin aging?

 

Depending on the degree of skin aging, different levels of severity and different types of wrinkles can be identified. To counter and slow down the degenerative processes related to skin aging, it is possible to intervene through restorative treatments and corrective treatments.

 

Restorative treatments

 

These are all those medical interventions that aim to reactivate the functionality of the skin responsible for the synthesis of collagen or fibroblasts.

 

First of all, we start from a stimulation of the STEM CELLS: these are totipotent cells, that are able to give rise to other cell lines with the ability to differentiate, that is reproduce and go to replenish both the skin and subcutaneous tissue.
With this type of therapy, it is possible to replenish all those areas of the face that appear empty and in which there is sagging skin.

 

Subsequently, a repair is stimulated by substances which, when inoculated at the dermal level, directly stimulate the cells by activating their metabolism (amino acids, growth factors).

There are many substances that are used to fill wrinkles, folds or furrows in the face, the so-called fillers, but certainly among the safest and most effective products we must mention hyaluronic acid. These are biocompatible gels which, when inoculated at the dermal level, can recreate the volume lost in an aged face, or can stretch the skin where there are more or less deep wrinkles. They are resorbable substances, so after a few months they are no longer present in the skin, but are capable, in the long term, of stimulating the production of rigid and fibrotic collagen. For this reason, their use must be limited to all those situations in which it is no longer possible to stimulate the skin through physiological processes and therefore are suitable for the restructuring of skin that is very damaged by chronoaging and photoaging.

 

  1. Many facial wrinkles are caused by the contraction of mimic muscles: frontal wrinkles, the so-called “crow’s feet” around the eyes, glabellar wrinkles (vertical wrinkles located between the eyebrows).

 

In these cases treating wrinkles with micro-doses of Botulinum Toxin is indicated. This substance causes a momentary relaxation of the muscle responsible for the wrinkle which therefore, for a few months, will no longer be present or will be considerably attenuated. As has been said for the fillers, the effect of botulinum toxin is also momentary, so it is necessary to maintain the results obtained with periodic retouching.
